St. Ann's Corner of Harm Reduction

Since 1990, we have operated the longest continuously running syringe exchange program in the United States. We provide compassionate, nonjudgmental support to people who use substances, whether they are navigating high-risk environments on the streets or seeking holistic health services at our center.

The Challenge

The South Bronx is at the epicenter of a deepening overdose crisis fueled by stigma and rooted in systemic inequities. Communities of color—particularly Black and Latinx residents—are bearing the brunt of this public health emergency. At our core, we champion harm reduction as a radical act of care, meeting people with empathy, mercy, and culturally rooted support. We believe everyone deserves access to life-saving resources, no matter their circumstances.

A Positive Community Force

SACHR has multiple teams providing support throughout the week at our main agency as well as on outreach and with our community clean ups. We offer a holistic network of care that includes HIV and Hepatitis C testing, safer use supplies, behavioral health support, and a range of harm reduction and healthcare services all grounded in compassion and respect. Our integrated approach promotes health, healing, and community connection for people navigating complex challenges. We also provide free benefits screenings to help participants access the resources and support they deserve.

A Sanctuary for Collective Healing

We believe everyone deserves access to safety, care, and dignity, no matter their journey. Our space is a sanctuary for rest, recovery, and connection, whether you need support for a moment or for the long haul. Guided by harm reduction principles, we meet people exactly where they are, on their terms and without judgment.

Our services are free, anonymous, and rooted in the belief that healing is a human right. We honor every step our participants take toward reclaiming their health, autonomy, and power. Healing isn’t a finish line; it’s a lifelong process supported by community and care.

Outreach Services

We’re here for you, wherever you are. Each week, we bring our services directly to the community through outreach locations, ensuring that everyone has access to the resources they need. By meeting people where they are, we create safe spaces for health, connection, and support on your terms.

From syringe access and HIV/HCV testing to safer sex supplies and PEP/PrEP counseling, we’re committed to supporting your health and well-being with dignity. We also take responsibility for keeping our community safe and clean by actively cleaning up syringe litter, ensuring that public spaces are free from hazards for everyone.
